Why the app never gives you the big one
TikTok shows profile pictures small because that is all a feed needs. The full-resolution file exists on their servers and is public, it is just never linked anywhere you can reach. This page asks for it and hands it to you.
The download goes from TikTok's own image servers to your device. We do not re-encode it, so what you get is the original file at the size the owner uploaded.
Before you use somebody else's
A profile picture belongs to whoever made it. Saving one to look at it, to check whether an account is impersonating you, or to keep a record, is ordinary. Reposting it as yourself, or building a fake account with it, is impersonation, and TikTok removes accounts for that.

Questions
Can I download a private account's picture?
No. Private accounts are not readable by anyone who does not follow them, and that includes their profile picture at full size.
Does the person know I looked?
No. Nothing about this leaves a trace on their account. It is the same public data anyone visiting the profile can see.
Why is the image sometimes smaller than expected?
Because that is what the owner uploaded. We serve the largest file TikTok holds, and if they uploaded a small picture there is no bigger version to find.
Do you keep the pictures?
We cache the profile data so the same lookup does not cost another request, and that cache holds the image link rather than the image. The file itself goes straight from TikTok to you.
